UpShot Health leverages the power of communities enabling employers of all sizes to benefit from dedicated care. This means that no company is too small to implement an UpShot Health solution. Employers pay a low, highly budgetable access fee for their population and receive a dedicated team of full-time, trained nurse practitioner professionals providing a broad range of health and wellness services. By sharing resources with other local employers, smaller to mid-size organizations receive the benefit of a full-time clinic at a fraction of the cost. Each clinic is served by an experienced registered nurse or nurse practitioner, and health coach to serve patients with personal attention. Our nurses act as health advocates for our members, helping them naviagate the complex health care system.

Population Health Management (PHM) is a key component of our practice management capabilities, and it drives our customized patient communication and engagement strategy. We view this approach to care customization as digital therapeutics. The outcomes of the PHM data guides the conversation with employers on the primary objectives to be achieved. Customized health care also means that our clinicians will follow-up with our members shortly after meds are dispensed by their primary care physician to ensure that there are no side-effects impacting adherence rates and medical outcomes.
